For Your Consideration

Physical Functions:

Independence: How important is it for you to be able to feed, bathe, and dress yourself?

Bodily Functions: Do you require control of your bladder and bowels to feel you have an acceptable quality of life?

Pain Tolerance: Is being as comfortable as possible a top priority, even if it means using medication that causes grogginess?

Mobility: How important is the ability to move around on your own?

Life Support: Would you find life unacceptable if you required mechanical ventilation or artificial nutrition and hydration indefinitely?

Cognitive Considerations

Cognitive Awareness: What is your threshold for cognitive function? For example, would you want to continue living if you could no longer recognize loved ones or understand your surroundings?

Memory:  Is preserving memory and the ability to interact verbally with others essential to you?

Communication: How vital is the ability to communicate your thoughts and needs clearly? 

Interactive, Social & Community Considerations

Social Connection: Would you be willing to live in a state where you could no longer interact with family and friends?

Environment: Is living at home and being able to participate in your community a priority?

Life Purpose: What gives your life purpose? Would you consider living under circumstances where you could no longer pursue those activities? 

Reflection Questions

Upon reflecting on the above considerations, answer the following:

  1. What ability(ies) can you not live without?

  2. What does quality of life mean to you? 

  3. When you’ve been ill or injured what has brought you comfort?

  4. What are you willing to go through for the chance of more time?
